E111 Insurance Form ExplainedE111 Insurance is free. You can get it from your local post office. Make sure you fill it in and give it to the post office to be stamped, signed and returned to you (if it is not signed by the post office, it is not valid). To complete the form you will need your national insurance number (you will find it on your payslip) or your pension number. Keep it with your passport and always carry it when travelling in the European Economic Area. E111 does not expire, but remains valid as long as you remain a resident in the UK. You will need to apply for another if you use it to make a claim for treatment or if you lose it, also if you change your address you should get a new E111. The E111 covers you, your spouse and your children (up to the age of 16, or 19 if they are in full-time education). It also covers an unmarried partner if you have children together. When children reach adulthood they should apply for their own form and you should get a new E111 without the children included, before your next ski trip.
